How our service works:
1. Contact us to schedule appointment. Will come to your home, job, or place of business.
(We also accept non-scheduled patients if not totally booked).
2. Let us know your preferred lab (i.e. Quest Diagnostics, Labcorp, UCI, etc.).
3. Email us your lab order, insurance card, and ID card. (If you don’t have an order your provider can fax it).
That’s it!
FAILURE TO PROVIDE REQUESTED DOCUMENTATION, APPOINTMENT WILL BE CANCELLED.
Mr. Vein charges a service fee of $70. The fee includes venipuncture, specimen processing, and delivery.
(This price is for patients within 15 miles of Buena Park, CA 90621. No service beyond 20 miles).
$60 service fee for patients with kits and/or Autologous Serum Eye Drops (With tubes and order). No service beyond 20 miles of 90621).
$40 specimen pick-up includes processing & delivery. (No service beyond 20 miles of 90621).
